Abstract: 

هدف از این تحقیق بررسی عوامل موثر بر مرگ و میر در بیماران مبتلا به مالاریای شدید بود. این تحقیق به روش گذشته نگر و از طریق بررسی پرونده های بیماران بستری در بیمارستان امام خمینی (ره) تهران و بیمارستان بوعلی زاهدان بین سالهای 80-71 انجام شد. 20 متغیر بین دو گروه Case ( 25 نفر مبتلا به مالاریای شدید منجر به فوت) و شاهد (52 مبتلا به مالاریای شدید) مقایسه شدند و معنادار بودن آنها از نظر آماری از طریق آزمون  (Logistic regression)سنجیده شد. براساس این تحقیق عواملی که باعث افزایش مرگ و میر در مبتلایان به مالاریای شدید می شود عبارتند از: سن بالا، جنس زن، ملیت ایرانی، کمای عمیق، هیپرونتیلاسیون (افزایش تعداد تنفس)، آنزیم های کبدی بالا، ازایش گلبول های سفید خون، طولانی شدن PT و کاهش بیکربنات خون شریانی. در این بررسی ارتباط معناداری بین خواب آلودگی، کمای سبک، درجه حرارتی دهانی، تعداد ضربان قلب، میزان هموگلوبین و هماتوکریت، کراتی نین سرم، بیلی روبین، پلاکت، گلوکز خون و فشار خون و سطح G6PD با مرگ و میر بیماران یافت نشد. ضمنا تعین ارتباط حاملگی و تشنج با مرگ و میر به علت موارد محدود قابل سنجش نبود.

Abstract: 

Background: Descemet stripping and automated endothelial keratoplasty (DSAEK) has gained widespread popularity in the treatment of corneal endothelial dysfunction. In the present study, we evaluated the Outcome of DSAEK in our center. Methods: This retrospective cross-sectional study was conducted on 60 patients, who had undergone DSAEK at least 12 months before in Khalili Hospital, Shiraz University of Medical Sciences. On follow-up, best spectacle-corrected Visual acuity (BSCVA), refractive errors and specular microscopy parameters were assessed. Results: The majority of the studied eyes (49. 2%) had a BSCVA within the range of 20/40 to 20/30. BSCVA was worse than 20/200 in 8 (13. 3%) patients; 6 (75%) patients showed graft failure and 2 (25%) patients had graft rejection. Mean post-operative astigmatism was 0. 29 1. 88 diopters. Themeanrefractive hyperopic shiftwas1. 30 1. 34 diopters. Specular microscopy indicated that 3 (5. 1%) patients had cell density of less than 700 cells/mm2, but in none of the patients cell density was less than 500 cells/mm2. Furthermore, hexagonality of endothelial cells in 50% of the eyes was 54% to 67%. Mean coefficient of variant was 27. 76%. Abnormal variations in specular microscopy findings were mostly confined to endothelial cell density, sparing other parameters. Conclusions: In general, results of this study demonstrated acceptable BSCVA in the average duration of one-year post-DSAEK regarding late complications such as graft failure, graft rejection and epithelial downgrowth.

Abstract: 

Purpose: To evaluate Visual Outcome of post traumatic endophthalmitis managed in Rasoul Akram hospital during 1989-1997.Materials and Methods: Hospital records of 147 cases of post-traumatic endophthalmitis were evaluated retrospectively. The cases had progressive anterior segment inflammation and vitritis after penetrating globe injuries. Effect of initial Visual acuity (VA), red reflex (RR), relative afferent pupillary defect (RAPD), and positive culture on final VA were determined.Results: The cases included 111 males and 36 females. Mean (SD) age was 18.4 (16.9) years. Initial VA of 92.5% of patients was below 20/200 and 7.5% of them had a VA more than 20/200 that increased to 33.6% after 2 months (P<0.0005). Relationship between initial VA and RAPD was significant (P<0.0001). Relationship between RR and culture with final VA was not significant.Conclusion: The Visual prognosis of patients with post-traumatic endophthalmitis is guarded. It seems that RAPD and initial VA are important prognostic factors on Visual Outcome. Case-control studies are recommended to increase treatment success.

Abstract: 

Introduction: Retinal vein occlusion causes sudden Visual loss and macular edema.Visual improvement after intravitreal Bevacizumab in recent years is argued in this study. To define the Bevacizumab effect on Visual outome in retinal vein occlusion.Methods: All attendants who had a diagnosis of retinal vein occlusion after ophthalmic examination enrolled in this study. The logarithm of the vision set, before 1.25 mg Bevacizumab injected intravitrealy was compared with postoperative Visual acuity of log Mar of 1, 3 and 6 months after injection. The possible side effects of intravitreal injection of Bevacizumab, after 1, 3 and 6 months were recorded. The Cross Kalvalys, Wilcoxon and Friedman rank correlation coefficients of  a £0.05 were assessed in study.Results: The mean age of 39 patients was 60.70±10.9 years. The mean disease duration was 2.29±2.72 months. There was statistically significant difference in mean Log Mar Visual acuity regarding the duration of time. Mean Visual acuity before injection was 1.58 ± 0.81, a month after the injection 0.93±0.77, three months after the injection 1.16±0.97, and six months after injection, 1.27±1.Conclusion: The Bevacizumab has significant role in improving vision in patients with retinal vein occlusion, although ocular complications after intraocular injection of Bevacizumab are far less than other similar approaches but few no responding occur which develop neovascular glaucoma.

Abstract: 

In this report we present a case of severe facial injury due to a donkey bite. Immediate repair with surgical flaps was performed. No significant complication was observed at the time of surgery or during follow up and the result was acceptable. Animal bites are relatively frequentandmostoften aredoneby dogs. Besides dogs, other animals such as cats, horsesanddonkeysmayberesponsible for this type of injuries. Although donkey bites to the facial area are very rare, they can cause severe and life-threatening injuries. Early management of facial injuries caused by animal bites is acceptable nowadays and guarantees satisfactory Outcome.

Abstract: 

Purpose: To assess the results of intracorneal ring (ICR) segments for treatment of patients with pellucid marginal degeneration (PMD) Methods: In this prospective interventional case series study, eight eyes of 5 patients with PMD who were contact lens intolerant received one or two Intacs (Intacs, Addition Technology) segments. Preoperative and postoperative evaluations included slit-lamp examination, uncorrected Visual acuity (UCVA) and best corrected Visual acuity (BCVA) and keratometry by Pentacam scheimpflug camera (oculas opticigerate GmbH). Patients were followed for 3 months and all parameters were reviewed at the end of follow-up time. Results: Three months postoperatively, UCVA significantly improved from preoperative values after Intacs implantation (mean 1.07±0.27 [SD] and 0.5±0.27 respectively). The BCVA also improved from preoperative state compared to 3 months after surgery (Mean 0.40±31 and 0.18±0.17 respectively). The BCVA was 5/10 or more in all 8 eyes. Spherical equivalent (SE) and refractive cylinder decreased significantly compared to preoperative values (Mean -9.404.98 to -6.53±5.00 and mean -6.93±3.79 to -3.06±2.07 respectively). Topographic cylinder decreased postoperatively, but this decrement was not statistically significant (mean -5.42±3.47 [SD] to -5.08±3.20). Conclusion: In our series of patients, ICR implantation proved to be a safe and effective method for correction of PMD and improving the vision in these patients.
